Glowplay Indoor Playground Play Area Rules

- All participants must always be accompanied and supervised by a responsible adult. - No bare feet allowed. Grip socks are required in all play areas. - No food, drinks, or gum are allowed in the play area. - No roughhousing, pushing, or shoving. - No climbing on or over safety netting, barriers, or outside play structures. - No throwing of any objects within the play area. - No outside toys or equipment are allowed in the play area. - GlowPlay Indoor Playground Ltd. reserves the right to deny entry or remove any participant who violates rules or behaves in an unsafe manner.

Glowplay Indoor Playground Liability Waiver, Release and Assumption of Risk

This waiver is required to enter GlowPlay Indoor Playground Ltd. ("GlowPlay") Please complete it in full. You must be at least 18 years of age in order to sign a waiver. If you are NOT 18 years old please have your parent or guardian sign this form on your behalf. By digitally signing this document below, you will waive certain legal rights, including the right to sue. **PLEASE READ CAREFULLY** 1. In consideration of GlowPlay Indoor Playground Ltd. ("GlowPlay") allowing my child(ren) to use the facilities, I acknowledge and accept all risks associated with the play environment, including potential injuries. 2. I warrant that I am the parent/legal guardian of the listed child(ren). By signing, I accept full responsibility for their safety and well-being. 3. I acknowledge that GlowPlay and its representatives are not responsible for supervising the children beyond enforcing safety rules. 4. I release GlowPlay from any liability, loss, damage, injury, or expense resulting from facility use. 5. I give permission for GlowPlay to authorize emergency medical treatment if necessary. 6. I confirm that my child(ren) are in good physical condition to participate. 7. I confirm that I am either the child's parent or guardian, or I have been duly authorized by the parent or guardian to execute this waiver on their behalf. 8. I agree that my digital signature authorizes GlowPlay to maintain this waiver for continuous, multi-use entry.
